All toolbars and the menu bar are missing when you start Excel 2000 / 2003

SYMPTOMS
When you start Microsoft Excel, the toolbars and the menu bar are missing. Also, you may not be able to use the right-click feature.
CAUSE
This problem may occur if the Excel toolbar file (Excel.xlb) is damaged.
RESOLUTION
To resolve this problem, follow these steps:
1. Close Excel.
2. Click Start, and then click My Computer.
3. On the Tools menu, click Folder Options.
4. Click the View tab, click to clear the Hide extensions for known file types box, click Show hidden files and folders, and then click OK.
5. Locate the Excel.xlb file in the following folder
C:\Documents and Settings\username\Application Data\Microsoft\Excel
where username is your user name.
NOTE: If Excel.xlb is not in this folder, search for it by clicking Start, and then clicking Search.
6. Right-click Excel.xlb, click Rename, type ExcelXLB.old, and then press ENTER.
7. Restart Excel, and then re-create your custom tool bar and menu bar settings as required.
